Description

This video depicts the feasibility of safe removal of fallopian tubes and ovaries, even at post-menopausal age, wherein the ovaries are atrophic and hence lateralised.

This may be required if there is a history of Cancer breast in the family.

The advantage of the vaginal approach over laparoscopy is that Expensive gadgets are not required, and the procedure can be completed under Regional Anaesthesia.

Sometimes, the left ovary is inaccessible and then one can always resort to Laparoscopy or, more recently, Vaginal -Natural Orifice Transluminal Endoscopic surgery( V-Notes ) surgery.

Important steps include:

1. Trendlenberg position
2. Packing of Small bowel withRoller gauge
3. Replace Landon retractor with a broader retractor like Purandare retractor
4. Take Round ligament separately
5. Use special curved pedicle clamp, devised by Dr Shirish Sheth.

Precaution: One has to be flush with the ovary on the Infundibulo-pelvic
