Ectopic pregnancy is often diagnosed early and amenable to medical treatment of standard criteria are met. Systemic methotrexate is the standard of care. Follow up is with beta HCG levels. A 15% threshold on day 4 after injection is useful for decision making.

It's not easy always to convince a patient regarding the mode of treatment. The patient and her relatives want a clear choice between medical and surgical and once you pronounce that medical treatment is the standard, they take a condition of ectopic very lightly; even go for litigation when you ask for surgery. Hence its always wise to keep surgical option ready to keep yourself safe. I fully agree the protocol of 4th day beta hcg estimation i.e. after 36 hours after the first estimation. We must be careful in those cases falling in the gray zone. Thank you very much.
