Hello Everyone! Pleased to share an interesting case done successfully at using the *Karl Storz Rubina 4K ICG System* 34 year old lady, severe pain during menses and extremely painful sexual intercourse. Consuming NSAIDs thrice daily for about 15 days a month MRI showed *Grade 4 Deep Infiltrating Endometriosis with rectal adhesions* to the uterus and ovaries, endometriotic infiltrates into uterosacral ligaments and mild hydroureter due to peri ureteric endometriotic infiltrates Cystoscopy and bilateral ureteric catheterization was done with injection of ICG dye. Using the ICG overlay mode, complete adhesiolysis with ureterolysis, dissection of para rectal spaces, peritonectomy, excision of USLs, and rectal adhesiolysis was done including excision of a full thickness vaginal endometriotic Nodule. *Ureter was seen lit up in green colour throughout the case* , enabling dissection millimeters away from the ureter without any risk of injury. After complete adhesiolysis, a stent was placed to prevent stricture due to post-op fibrosis Interestingly, the ICG Overlay mode is even better than using the green ICG glow on a monochromatic black and white background where one has to switch back and forth between colour and monochrome to visualize the ureter first, and then dissect without ICG aid on colour background Dr Sanket Pisat

Q) Sir, I bought Hegar's hysteroscope, the notch on the monitor screen is on opposite side of light cable, so while entering the cervix should the crescent on the screen be on opposite side of the notch?

Q) Sir, hysterosscopic TCRF, with fluid deficit 1.2L, and we has to stop the procedure, she has been adviced for a re look Hysteroscopy +\- TCRF in 6 weeks time. There was perhaps about 10% of the sub mucosal fibroid which could not be removed due to the high fluid deficit. She is 27yrs old nulliparous. Is it the right advice for a telomeres Hysteroscopy!? And in the interim can she tey for a pregnancy?

A) 1. Deficit of 1.2 L of saline is not an indication to stop surgery 2. You can do repeat surgery in 6 weeks no problem 3. What is telomeres hysteroscopy? 4. No it is better to remove the myoma first
